  • Social Security cards for everyone in the household
- Insurance Cards
  • Photo Ids for everyone 18 years old and older
  • Birth certificates for anyone 17 and younger
  • Written proof of income (wages, child support, TNAF, disability, unemployment, Social Security, SSI, SSDI
  • Lease
  • Eviction notice
  • Current bank statement(s).

Rapid Re-Housing Program assists with obtaining permanent housing. Household income must be at or below 30% of Average Median Income and customers must present the last 13 weeks of income, and photo identification.
