Reside in Logan County, be pregnant or post-partum (up to 6 months), be breastfeeding an infant (up to 1 year), have an infant or child under the age of 5, earn a low or moderate income. A person receiving Medicaid, Food Stamps or cash assistance automatically meets the income eligibility criteria.
