Eligibility

  • Serves caregivers with legal custody or guardianship of a child whose birth parents cannot care for him/her.
  • Child and caregiver must live in Portage County.
  • Family (including child) must have income no greater than 300% of the Federal Poverty Guideline Level.
  • Custody must have been awarded on or after July 1st, 2005.

Required documents

  • Verification of legal custody or guardianship.
  • Proof of income.
  • If needed, Agency will help clients pay for any required documentation that the client does not already have.
