Eligibility
Open to Medicaid recipients who have Molina/Passport as their State Medicaid Managed Care Organization (MCO). Rewards must be claimed within 90 calendar days of receiving the qualifying service and individuals must be enrolled with Molina/Passport Medicaid at the time of the service and the time of claiming the reward.
