Eligibility

Open to members of Buckeye Medicaid age 18 or older.

  • Destinations must be less than 30 miles from the member's home to be eligible.
  • Assistance is limited to 15 roundtrips or 30 one-way trips per year.

Application process

Contact Member Services at least two business days (48 hours) in advance to schedule a trip. Note that this does not apply for urgent needs (such as same-day or next-day visits) or hospital discharges.
