Passport Medicaid Waiver Program | Central Ohio Area Agency On Aging
Provides home care to older adults whose care needs are the same as those in a nursing home. PASSPORT's goal is to provide an alternative to nursing home placement, and keep older adults in their own homes. Services available include homemaker, transportation, personal care services, home-delivered meals, emergency response systems, adult day services, and case management. In addition, those who are eligible for PASSPORT receive a Medicaid card to cover the cost of medications and health care. To be eligible, a person must be 60 years of age or older, meet both the financial requirements and have qualifying ADL/IADL functional impairments, as well as other admission criteria.

COAAA plans, funds, and delivers services that help older adults and individuals with disabilities remain safe and independent in their homes. With the assistance of area providers, COAAA arranges and coordinates services to help people with activities of daily living in the home, such as homemaking, home-delivered meals, personal care, transportation, and more. COAAA offers education and resources to caregivers, professionals, and the public. COAAA also advocates for programs and policies that benefit older adults and individuals with disabilities.
