Eligibility

  • Lucas County households with a gross income at or below 175% of the federal poverty guidelines and meet at least one of the following criteria listed below:
  • Household member 60 years of age or over
  • Household member with documented medical conditions verified by a Licence Medical Professional who is qualified under Ohio State Law to write prescriptions (must present verified documentation from your physician or registered nurse practitioner. Documentation must state: "Due to a chronic illness, this patient would benefit from continued electric service and/or air conditioning.")
  • Household member with electric utility service disconnection notice, has been shut off, or new electric service is being established for the household
  • The maximum benefit amounts for program are $500 for customers of regulated utilities and $800 for customers of unregulated utilities (municipal or cooperatives). Air conditioning units and/or fans may only be provided to eligible customers who have not received an air conditioning unit or fan in the last three years.

Required documents

  • Current gas bill and/or account numbers
  • Social Security numbers for all household members
  • Proof of US Citizenship for all household members (birth certificate, baptismal records, US Passport, voter registration card, Military Service Record, Verified Citizenship for OWF Program, Indian Census Record, proof of Legal Resident/Qualified Alien- Naturalization Papers/ Certification of Citizenship, INS ID Card, Alien Registration Cards/ Re-entry permits, Permanent Visa, INS Form
  • Proof of Income for the entire household for the past 30 days for all income types, except self-employment and seasonal income
  • Self-employed and seasonal households must provide 12 months of documents
  • A current notarized letter is required for individuals applying on the behalf of another applicant
  • Documentation of co-payment, if required
  • Proof of disability
