Application process

Call the office or email for information or to schedule a ride. Transportation must be requested at least 10 days in advance. If emailing or leaving a message: client must leave name, phone number, Medicaid Case number (on Medicaid Card), date and time of appointment, Doctor's or medical facility name and address. Will also need to leave either Social Security number or Medicaid Case number (on Medicaid Card) when leaving message.
