Head of household must be 55 and over (spouse is able to be under 55). Annual income limit $27,900.00 for household of one, allowable income limit increases based on household size.

Once application is complete, you will need: Photo ID, birth certificate, Social Security Card, and proof of income. Social security award letters must be less than 120 days old.
