$25 fee for certified copies. We do not accept debit or credit cards: cash, money orders or personal checks are accepted. Please make check or money order payable to: Guernsey County Health Department.

Guernsey County Health Department can now provide you with a birth certificate if you were born in the state of Ohio. In 2010 the health department was granted access to provide birth certificates from the Ohio Department of Health statewide issuance site. Death certificates can only be issued from our agency if the individual passed away in Guernsey County.
