Eligibility

Although there are no age restrictions, this service is targeted to physically and/or cognitively impaired older adults. Will serve those with Alzheimer's/dementia, provided they are not at risk of harming themselves or others.

Application process

Phone M-F: 9:00 am-4:30 pm for information and to schedule an on-site or at-home intake. The initial screening is conducted by phone.

Fees

Fixed fee. Accepts third-party payments, Passport, My Care Ohio, Carstar, and some insurance providers.

Description

Offers programming and services for individuals living with dementia, and for physically or cognitively challenged adults. Provides regular, ongoing daily care as well as short-term respite care. Offers medication management, with meals that are catered to each individual's needs and overseen by a dietician. Provides both social and medical model of care.
