- Provides case management services to eligible individuals, ages 3 and over. - Services include service coordination for eligible individuals, and determination of eligibility for services of the Lorain County Board of Developmental Disabilities. - Performs the following case management activities in addition to others: complete assessments; identify resources that are available to individuals; develop, review and revise individual service plans; facilitate provider selection; secure and verify provider commitments; facilitate effective communication and coordination; conduct actions related to Medicaid; conduct service monitoring; and remediate health and welfare concerns.

- Call the Service and Support Administration office to begin the intake process. - An initial phone interview is conducted to determine if there is a qualifying diagnosis prior to proceeding with the remainder of the eligibility determination and intake process. - If eligibility status needs to be determined, applicants can expect a determination within 45 days of referral and receipt of required documentation.
