Photo ID (Mason City School ID or current report card for all school-aged children),Birth certificate (Birth certificate for all non-school aged children),Other documents (Current hotel invoice (if applicable), Eligbility to Take Food Home/Income Certification form (signed at each visit)),Proof of residence (Proof of residency for all household members over the age of 18 (state issued photo ID, driver’s license, or Green Card and a utility bill or piece of mail received at current address within the last 30 days))
