Ryan White Emergency Assistance - Ohdap - Adap | Equitas Health
Provides medical case management and help with HIV related medical, dental, and mental health visits and payments. The OHDAP (Ohio HIV Drug Assistance program) is also coordinated through our agency. This program provides financial aid for HIV related medications and some medical insurance premium payments and co-pays.

Application process
Call to complete Initial Intake over phone. After that a Medical Case Manager will coordinate program enrollment by scheduling a time for phone based, agency based, or community based appointment. Successful program enrollment requires documentation of HIV, income (pay stubs) or no income statement, proof of Ohio residency (Driver's license, state ID, utility bill). There are special enrollment procedures for people without documentation (undocumented, homeless). Initial Intake takes about 20 minutes First full program enrollment takes about 1-2 hours.

Documentation of HIV, restricted to incomes limits of 300% Federal Poverty Guidlelines, documentation of income (pay stubs) or attestation of no income statement, proof of Ohio residency (Driver's license, state ID, utility bill).
